MAIN STEPS OF THE JOBPOTENTIAL HAZARDSCONTROLS
Break the job down into steps. Each step should accomplish a specific component of the job Identify the hazards associated with each step i.e. what can go wrong? Actions or procedures to eliminate or minimize the risks
Ensure to stay in vehicle as straddles & forklifts are in the area Ensure that flashing hazard light is connected & working
Discuss access hazards of the job Making sure that the job is clear of operations
You could be hit, blinded, burnt or crushed foot, hearing damage Ensure that all personal are wearing their PPE gear
To stop access to area for unauthorized personal Secure area with barricades
Near miss, incidents, injury, Death, Fire Issued & current before commencing
People moving machinery without authorization Clearly tag out machinery
Slipping & dropping of tools Access vehicle in a safe working manor (use steps)
Fix hazards when they arise Beware of & identify potential hazards
All tools & equipment are dangerous if not used in the correct way Use as per manufacturers instructions
Yes No Electrical equipment is dangerous if exposed wires or damages aren’t addressed. Possible Electrocution, electrical fires, short circuits, electrical burns. Ensure Leads are free from damage, no exposed wires, PAT tags are current in date and visible.
Yes No Falling Safety harness must be worn in cherry picker. Ensure lanyards and harnesses are visually inspected before each use. Inspection tags are visible and in date. Trained licenced operators only to operate.
Yes No Fires, explosions, burns Oxy/acetylene bottles are to be upright and secured whilst in use. Visual inspection of burning gear before each use. Flashback arrestors are to be in place. Firefighting equipment to be at hand.

Sparks, grit and hearing damage

Safety goggles/helmets/glasses must be worn Adequate hearing protection to be used. Task specific PPE
Falling Safety harness must be worn above 1.5m, Ensure safety chains, lanyards and harnesses are visually inspected before each use. Inspection tags are visible and in date.
Tripping & falling Keep good housekeeping of work area
Things left around the area ie. Tools & equipment Ensure all tools & equipment are accounted for also clean & sweep area
Machine cannot be moved Make sure tags are removed from machinery
If not ok follow JSA again Follow JSA
Emergency evacuation - management sends someone to clear us from area when we have already left Ensure that management are aware of job completion & tax invoice has been sign for
Fire, burns, explosion Before leaving workshop each day:
At Boncon's workshop generator is to be turned off for a minimum stop time of 15 minutes and cool enough to touch. Check personal clothing is not smouldering/hot with embers.
No hot works to be carried out within a 15m radius. Fire extinguishers to be within arm's reach.
Nozzle to be securely attached to jerry can and checked before commencing the refuelling.
All overspills to be cleaned up prior to starting generator. Fuel tank and jerry can lids to be closed, securely fastened and checked.
Fire, burns, explosion Onsite ensure generator is turned off for a minimum stop time of 15 minutes and cool enough to touch. Check personal clothing is not smouldering/hot with embers.
No hot works to be carried out within a 15m radius. Fire extinguishers to be within arm’s reach.
Nozzle to be securely attached to jerry can and checked before commencing the refuelling.
All overspills to be cleaned up prior to starting generator. Fuel tank and jerry can lids to be closed, securely fastened and checked.
Fire, burns, explosion Ensure it is permissible to refuel jerry cans and tanks on the back of a vehicle at selected fuel station. If not remove jerry can and place on ground.
Discharge any static before removing lids by touching vehicle panels.
All overspills to be cleaned up prior to starting generator. Fuel tank and jerry can lids to be closed, securely fastened and checked.
